The App is a self-help application. We encourage you to consult your health care provider before using this or any other self-help application to determine if it is right for your needs. The App and the services are not intended to be used for treating or diagnosing any medical condition.
If you are in need of medical treatment or emergency care, OR experiencing severe mental or emotional distress or have worsening of those symptoms, seek care immediately by contacting your healthcare provider or by going to the emergency department of a nearby qualified healthcare facility. In the event of an emergency in the United States , call 911 right away to reach your nearest first responder. The services do not provide medical treatment, medical advice or psychological therapy services, and Thrive 4-7 is not a healthcare provider. Additional mental health resources are listed on the Resources page in the menu page of the App.
Collection of Personal Data
Our Sites allow you to contact us and provide us with your email, your name, payment information and supporting data, and other information you may share within a contact form and email you send to us. Otherwise, our Sites do not collect any personally identifiable information from users. This allows you to visit our Sites without telling us who you are .
We also collect information you provide voluntarily in free-form text boxes on the Services and through responses to surveys, questionnaires and the like.
When using the App, we will ask you for certain personally identifiable information necessary for use of the App and for communicating with you, in addition to payment information unless the App is provided by an employer, health plan, or other arrangement The data collected includes information about you that can be used to contact or identify you, and information related to your use or potential use of App (collectively, “Personal Information”). Personal Information that we collect from you may include the following:
- Employment status
- Zip code
- Phone number
- Level of education
- Relationship status
- Personal interests
- Credit card or other billing information(where applicable)
- Email address
- Veteran status
- Any other information or data that you provide when using the App.
A cookie is a small data file that certain websites write to your hard drive when you visit them. A cookie file can contain information such as a user ID that the site uses to track the pages you have visited, but the only personal information a cookie can contain is information you supply yourself. A cookie cannot read data off your hard disk or read cookie files created by other websites.
Use of Tracking Systems on the App
We may use data files written to your device when you use our App to track certain ways you use the App.
We may also collect device identifiers of devices using the App. We may use the information to aggregate information about the number of users of our App, App usage and from where our App is accessed. We do not generally use device identifiers to track personally identifiable information. We reserve the right to use device identifiers to identify persons who abuse our App, or threaten our agents, employees, or other users. We may also use other tracking technologies, but do not intend to use such technologies to collect personally identifiable information.
Use and Disclosure of Information
Comments or questions sent to us using email or secure messaging forms through the Services will be shared with our staff that is most likely to be able to address your concerns. We may archive your messages once we have provided you with a response. If we find your personal question valuable to community discussion, we reserve the right to anonymously post your question on the Services by using all reasonable efforts to remove any names or personal identification, including photos and username. Please note that email communications will not necessarily be secure; accordingly, you should not include credit card information, PHI, or other sensitive information in your email correspondence with us. Thrive 4-7 may aggregate your information with other users’ information to determine and distribute aggregated statistics and information about the Services and its use and users unless you opt out of sharing aggregated data as permitted in the App. This aggregated information will not identify any specific user.
Thrive 4-7 may also disclose personal information:
• when we have your permission to do so,
• to companies who work for Thrive 4-7 to provide one or more parts of the Services or that provide related administrative functions (such as support services, analytics, and fraud detection). These third parties may have access to your personal information; if they do, this access is only so that they may perform these tasks on our behalf and they are obligated not to disclose or use it for any other purpose; or
• when we believe in good faith that disclosure is reasonably necessary to (1) comply with a legal process or law, or to respond to a subpoena, court order, or government request for information, (2) enforce the Terms or other agreements, (3) respond to claims that any content on the Services violates the rights of third parties, or that otherwise arise from your use of the Services, or (4) protect the rights, property, or personal safety of Thrive 4-7, its users, or the public.
• when required or permitted to do so by law for public safety. Disclosures may be made to protect you from a serious threat to your health or safety or to protect the health or safety of another person.

Access to Account
If you are a registered user of the App, you can change your account information through the Manage Profile page under Menu in the App. You may need to supply certain information so that we can try to verify that you have authority to modify this information.
We provide you the opportunity to opt-out of receiving certain communications from us, by emailing us at connect @thrive47.com. In addition, e-mails from Thrive 4-7 will provide you with the opportunity to unsubscribe from future mailings. Even if you opt out of such mailings, however, we reserve the right to send you e-mail and other communications to disclose maintenance and other service-related issues.
We intend to protect your privacy to the fullest extent possible as described in this policy while also fulfilling our responsibility to uphold all applicable laws and regulations. Due to potential legal requirements and other possible circumstances, we cannot ensure that all of your private communications and other personally identifiable information will never be disclosed or accessed in ways not otherwise described in this policy. We may be compelled under law to disclose information to government or third parties under certain circumstances or third parties may unlawfully intercept or access transmissions or private communications. Further, we can, and you hereby authorize us to, disclose any information to law enforcement or other government officials that we, in our sole discretion, believe necessary or appropriate in compliance with the law and/or government requests.
We have implemented reasonable and industry standard security measures on the Services to help protect against the loss, misuse and alteration of the personal information under our control. Additionally, we take reasonable steps to ensure that our third party business partners, including our hosting partners, provide sufficient protection for personal information.
You acknowledge that no method of transmission over the Internet, or method of electronic storage, is 100% secure, and that we cannot guarantee absolute security. If you have any questions about security on our Services, you may contact us. We will promptly make any legally required disclosures of any breach of the security, confidentiality, or integrity of your unencrypted electronically stored “personal data” (as defined in applicable state laws on security breach notification) to you as required by law.
Privacy of Children
Children under age 18 are not permitted to use the Services. We will not knowingly collect personally identifiable information on the Services from children under 18. If we become aware that a user of the Services is under the age of 18 and has provided us with Personal Information, we may delete such information from our files and may deactivate the related account.
This policy may be revised by us from time to time. By using the Services you agree that we have a right to change our policy without prior notification. The revised policy will appear on our app and/or web site with the date of last revision. Your continued use of the Services following such revisions will be conclusively deemed acceptance of any changes to these Terms.
